The Health Navigator program trains and certifies cultural leaders as Community Health Workers (CHWs) to support refugee families in navigating our complex healthcare system and to partner with healthcare providers to increase equitable access to healthcare services and resources.
enhancing community well being |
Health Navigators, as Community Health Workers (CHWs), support refugee families by addressing social determinants of health. In partnership with Cincinnati Children’s Hospital Medical Center Fellows, Community Navigators, and Re:Source Cleveland, they work to improve health literacy among refugees across the region through multilingual videos you can access below.
Additionally, Health Navigators provide culturally responsive, trauma-informed training and consultation to healthcare providers, ensuring comprehensive and compassionate care. |
success & outcomes |
RefugeeConnect Health Navigators are certified as CHWs by the Ohio Board of Nursing. In their first year, Health Navigators connected 100 individuals to health insurance, primary care and specialized health care, and supportive resources to address other social determinants of health.
